New Customer’s Requirements – Form 1583 |

Important: All new customer’s must complete the steps below within 30 days of
activation.
Due to USA postal regulations all new customers will not be able to order any
shipments or discards until the steps below are completed. Please complete
within 30 days of activating your account.
This is a one time requirement by the USA government and we can’t waive it.
You may complete these requirements by mail (courier) or if in the Houston
area, you may complete by person). For customers in Mexico, we also have a
Mexico City office where you can complete these requirements.

The steps are:
1) Complete, sign two (2) originals of
Form 1583.

Have an attorney, notary, bank or other official, sign and seal the form.
2) Make legible photocopies of two identification documents and both sides of
the credit card that you used when registering. See the instructions for
examples of acceptable identification documents.
